Online Forms Directory

The forms listed below will help you, as a tenant of Bluegrass Station, take advantage of some of the services we offer.

We will continue to add to and enhance this section to make working at Bluegrass Station easier for you and your employees!

Should your facility need a "tune up", feel free to take advantage of Bluegrass Station's maintenance services by completing a Work Request Form (PDF - 70kb).

Follow these instructions once you have accessed the form:

1. Fill in the agency or company name, building number, location in the building, your name, a contact name (if different), and a contact phone number.
2. Type in a detailed description of the problem. (If you need more space, attach additional pages.)
3. If you require an estimate before we begin the work, click the "yes" box, otherwise, click "no".
4. Print the form, sign it, and date the first signature lines.
5. Fax, hand carry, or mail the form to the Director's Office at Building 18. (See fax number and address to the right.)

If an estimate is requested, we will list the price and return the request to you for an approval signature. Work will begin once you have approved the estimate.

Are you interested in using the Bluegrass Station Fitness Center?  If so, please access a copy of the Fitness Center Waiver (PDF - 58kb). 

1. Complete the form, except for the signature line. You may complete the form online, then print it or you may print the form, then type or write in the information.
2. Bring the form to the Director's Office at Building 18.  Sign the form when you have arrived at the Director's Office and submit it to the Office Coordinator.     

Each person wishing to use the Fitness Center must have a signed and witnessed waiver on file in our office.
